Samantha Dennett Kozak, 59, Elkhorn, passed away at her home on Tuesday, April 30, 2019.

Samantha was born on July 16, 1959 in Cook County, IL to Howard and Patricia (Greene) Johnson. She graduated from Lake Zurich High School in 1977. On August 8, 1987 she married Edward Kozak at the Long Grove Community Church, Long Grove, IL. Sam was such a loving mother, grandmother, a great sister and has been such a good friend to others over the years. She enjoyed cooking, baking and going to the zoo. When Sam was younger she had a great love of horses and riding them.

She is survived by her husband, Edward; children Lexi Kozak, Whitewater, Oliver Kozak, Elkhorn; granddaughter Jade Sulser; sister Sarah Johnson, Lake Geneva. She was preceded in death by her parents.

A Celebration of Samantha’s life will be held at a later date.
